Those molecules might be fluorescent tags, that could allow visualization of drug localization, or polymer chains that could restrict the distribution of the drug to certain tissues or cellular locations. You could also attach radioactive isotopes to these drugs that could be used for either diagnostic imaging of potentially responsive tumors or as therapeutics capable of killing those tumors.
